Laser-Surgery in Otolaryngology







Abstract
Since the first laser which had been a product of modern science was developed by T.H. Maiman in 1960, it has been utilized in many other parts such as optics and technology, but it was in the 1970¢¥s that it was introduced to medical science. Since that time, it has been used as a new surgical tool owing to many experiments and studies. The laser is light amplification by stimulated emission of radiation. Clinically, CO2 laser and argon laser are useful.
The effect of the CO2 laser to tissue is not only to vaporize the fluid in the tissue in a moment, but also ignite and scatter the solid element by the pressure difference resulting from vaporization. As it occludes the vascular and lymphatic channal around the crator, so it can prevent bleeding and spread of the cancer cells.
Laser surgery has many remarkable advantages which are an extraordinary hemostatic effect, maximum operative field, little postoperative edema, very rapid healing of the wound, and minimal postoperative scarring.
We performed laser surgery in 27 patients who have diseases in otolarygologic field from April, 1982 to September, 1982, then we concluded as follows:
1. CO2 surgical laser is a practical instrument for incision or excision of tissue.
2. The microsurgical laser attachment to operating microscope allows laser surgery of the vocal cords to be carried out with exquisite precision.
3. We experienced the advantages of CO2 laser as follows; Tissue destruction is precise, immediate, and bloodless. Tissue manipulation is unnecessary and the absence of instrument means that visual access to the operative field is not obstructed. Tissue destruction is not complicated by edema, and healing is rapid. There is also very little, damage to normal tissue, with minimal tendency to scar formation.
4. Further application of laser surgery in otolaryngology will be forthcoming as experience and expertise accumulates.
